Confirmation Name Report
For the name you wish to take as part of your reception of the Sacrament of Confirmation, you may:

• Choose to take again your baptismal name (it must be a name of a recognized saint of the Church)

• You may choose a new name but it must be the name of a recognized saint of the Church or a title of Jesus or Mary. 

1. The name of your saint, the saint’s feast day and if the saint has been named a patron for a particular group of people or cause. 

2. Prayer to your saint (Many saints have special prayers that are written specially to ask for the Saint’s intercession.  Please include the prayer in your report.  

3. A couple of sentences about the saint’s background i.e., where the saint lived, saint’s family, saint’s occupation/ministry. 

4. Three to five sentences about the saint’s life and/or death that showed the virtues this saint practiced.  Name the virtue/virtues and an example from the saints life.  

5. Tell how this saint is a model for Christians today.

6. Tell what aspect of this person’s Christian life attracted you, or if you chose the saint’s name because of someone you admire, tell why you admire or admired this person.

7. The resource(s) you used to gather your information.
