New Parishioner Registration & Parishioner Update
Thank you for your inquiry about registering at Church of the Nativity. If your physical address falls within the boundaries of our parish, and you would like to officially register, please download and complete the following form:
After printing out the forms and filling them out completely, please return it to Karen in the Parish Office via fax, 913.491.5065 or mail it to: 3800 West 119th Street, Leawood, KS 66209.
Please also complete the Nativity parishioner Ministry Sign-Up Time, Talent & Treasure form either electronically or by filling out a hard copy. Click on ONE of the following links to complete your Ministry Sign-Up.

If you are inquiring about registration in the parish and you DO NOT live within the parish boundaries (see Map & Boundaries link), please call Karen in the Parish Office BEFORE you complete the on-line registration. She will be happy to discuss this with you.
Registration forms are also available in the Parish Office during regular business hours (8:30 am to 4:30 pm, Monday through Friday). You are welcome to come in and fill out a form in our office. |
